Store all chemicals in a location far away from foods and food containers.

Observed gasket on the refrigerator worn out and gunked up. Item needs to be replaced. Corrected on site. Item cleaned, to be replaced at a later date. Observed back door left open with secondary screen door broken and worn out. If screen door is to be used for ventilation, it needs to be fixed or replaced so as to not allow pests through.

Comments: Observed several food items not labeled or labeled incorrectly. This included sausage, egg and cheese burritos and sliced tomatoes (four-hour hold time items). Items that are time controlled must be labeled to indicate how long they have been kept out of temperature. Corrected on site. Time controlled food items disposed of and replaced with new stock from refrigerators. Observed several food handling employees wearing watches. Employees that handle food are limited to wedding bands and not jewelry that can potentially create cross contamination and prevent adequate handwashing. Corrected on site. Employees notified about improper jewelry. Observed floors containing fries, hamburger buns and chicken nuggets. Floors must be kept clean and free of potential food contamination sources. Corrected on site. Items picked up. Observed numerous flies in the kitchen. Problem seems to be the result of inadequate temperature control in kitchen.

Comments: Observed employee with an open plastic cup in the food preparation area. All personal cups must have a lid and straw. Corrected on site. Employee/owner informed. Observed establishment not having a bodily fluid clean-up kit on premises. Food establishments must have a way to clean up an event that reduces the risk of spread of an outbreak type event. These kits are required for all restaurants. Observed several plastic spray bottles and a large Ace brand container containing an unknown chemical solution. All containers used hold or carry chemicals must be labeled. Corrected on site. Items labeled and relocated. Observed pre-cooked bacon being stored within a rag inside one of the preparation coolers. Food must be stored in a way to protect it from contamination. Rags are not an appropriate means of storage. Observed back door open with screen door separating the outside from the inside. Door contained holes greater than a half inch and must be re-patched.

Items moved to freezer until temperature fell within proper cold holding limitations. Observed holes along the back wall corner leading to the outside and unsealed spaces along the floor in the dry stock area. Repair and seal immediately. Corrected on site. Owner is sealing currently. Observed numerous dead roaches throughout the kitchen. Corrected on site. Restaurant is currently in transition to new pest controller.

Comments: Observed an overall lack of managerial awareness and control with regard to temperature control. Cooling, reheating, cold/hot holding are all responsibilities of the person in charge. These items are to be checked on a constant basis by a knowledgeable individual. Observed many items in preparation coolers above 41 degrees. All cold time/temperature controlled foods shall be held at or below 41 degrees. Call refrigeration professional for equipment maintenance. At least three coolers appear to need maintenance. Repeat violation. Observed rice, pork and chicken below 135 degrees. All hot held time/temperature controlled foods shall be held at or above 135 degrees. Have a professional check all hot units to be sure that they can maintain proper temperatures. The steam units appear to be able to maintain the correct temperature, but not the small hot unit with the sliding door in the front preparation area. Repeat violation.
